First Visit

All new patients are introduced to our practice through a comprehensive examination done by Dr. Uyesugi.

This initial visit will be comprised of an oral cancer screening, an examination of your gums to diagnose periodontal diseases (also known as gum disease), and the temporomandibular joint to evaluate any abnormalities, and an analysis of your overall dentition for existing restorations, active decay and occlusal wear.

Your second visit will consist of a consultation so that we may understand what you need and want your needs and your wants and discuss with you your treatment options. If you have an emergency we will treat it during your first visit.

You may pre-register with us by using our secure online Patient Registration Form. This will save you time when you come in to our office for your first visit.

Please assist us by providing the following information at the time of your consultation:

• Any recent checkup or full mouth X-rays (if applicable).
• A list of your current medical prescriptions.
• If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
